We set appointments Monday, Wednesday through Friday, from 10 AM to 5 PM, Tuesday 10am to 7pm and on most Saturdays, from 9 AM to 1 PM. To schedule an appointment, please call (630) 262-2515. To request an appointment online, please complete and submit our Interactive Appointment Request Form below. Please do not use the Interactive Appointment Request Form if you have an urgent problem or to re-schedule an existing appointment. Instead, please contact our office at (630) 262-2515.
